Please Note:

The rewarded Recipient of this scholarship will receive the awarded funds once proof of enrollment to college, community college, vo-tech, or trade school has been presented no later than august 15th of the high school graduating year. Failure to show secondary education enrollment for fall semester will result in the runner up recipient receiving the awarded funds. Awarded funds are allocated to half the award per semester. Proof of enrollment for spring semester must be shown by the last day of February to receive the 2nd half of the awarded funds.

Evaluation by school official

Mandatory

A school principal, counselor, or advisor must answer the following questions regarding the students history and potential. the school official may photocopy this section, complete it and give it to the student in a sealed envelope, or respond on the original form and return it to the student.

The student has chosen appropriate course of post-secondary study?

The student consistently applies him/herself to the level of his/her potential?

The student cares about his/her family, school and community and shows an appropriate level of involvement?

The student shows maturity and good judgement in goal-setting and attachment efforts?

The student is resourceful and innovative, and displays good utilization of available resources?

School transcript Data

High school students and graduates with less than one year of post-secondary education must include a high school transcript and have school official (principal, Guidance counselor, or Advisor) complete the following section.

For students completing ACT please put "0" in the SAT section and vice versa for completing SAT
